Radja Nainggolan was born on May 4, 1988, in Antwerp, Belgium. He was raised by his parents, Marius Nainggolan and Lizi Nainggolan. He has two siblings: a sister named Riana Nainggolan and a brother named Manuel Noboa.

In 2010, Nainggolan was loaned to Cagliari, making his Serie A debut that year. In 2014, he was loaned to Roma, where he made his debut in the Coppa Italia. He later signed permanently with Roma.
